Provider Health Services (PHS) is seeking a highly qualified and compassionate Nurse Practitioner to care for geriatric residents in a skilled care community.

This is a full time position with a robust array of benefits, CE allowances, in house Web Casts, vacation, liability insurance and a competitive compensation package. We are seeking ANPs, FNPs, GNPs or ACNPs who desire a comprehensive practice in the specialized area of geriatrics. PHS develops the practice with the Skilled Community and physicians, while the Nurse Practitioner implements that practice with the support of the PHS corporate team, who provide billing, coding, credentialing, clinical and educational support. The Electronic Medical Record allows the Nurse Practitioner to accurately document according to CMS guidelines, while being able to spend quality time with the resident and family. This technology also affords our employees more personal quality time.

Our Nurse Practitioners enjoy a collaborative practice model of care with the primary physicians caring for these residents. They are also able to interact and educate staff, participate in the community’s QI program, collaborate with consultant pharmacists, registered dieticians, and the rehabilitation team of specialists. In this role, the Nurse Practitioner impacts the transition of care from the acute care setting and into the home setting, assuring the highest quality of continuity of care.
