Primary Responsibilities:
- Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake
- Ensures compliance with all state, federal, and JCAHO referral/intake regulatory requirements
- Establishes and maintains positive working relationships with current and potential referral sources
- Ensures seamless transition of patients to hospice by providing telephonic patient/family education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers
- Gathers, collates, and reports referral statistics including key customer referral trends; responsibilities shared between Intake Coordinator and the Office Supervisor
- Maintains an accurate listing of current daily census including activities related to admissions, discharges and transfers
- Identifies opportunities for additional or improved services to meet unmet customer needs
- Maintains comprehensive working knowledge of the hospice contractual relationships and ensures that patients are admitted according to contract provisions
- Completes all job duties in a manner that meets the accepted standards of practice and the Evercare Hospice & Palliative Care Policies and Procedures
- Contributes to achieving the company's mission
- Works collaboratively with others to achieve goals

Qualifications:
- Bachelor's or Associate degree in Business Administration or equivalent experience
- Strong computer knowledge and experience
- Experience working in a busy office
- Hospice experience
- Microsoft Excel experience
- Medical Billing or Insurance Billing experience preferred
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
- Excellent oral and written communication skills
