APN/PA to Perform House Calls in Camden County

Primary Responsibilities:

• Initiate and complete detailed patient assessments & physical exams
• Conduct wellness exams and sick visits
• Evaluate and manage common and acute chronic diseases
• Order, interpret, and evaluate diagnostic and lab results
• Refer and collaborate with other providers
• Prescribe pharmacological and non-pharmacological therapies

Requirements:

• Computer literate & EMR proficiency
• Unrestricted APN license in the State of NJ with at least 1 year of clinical experience
• Current DEA/CDS
• Willingness and ability to work independently
• Personable, passionate, and energetic
• Ability to travel within Camden County
• Previous experience with Medicare-eligible patients is a plus
• Bilingual a plus
