Care Coordinator - Good Samaritan Hospital

Schedule and Location Hours: Full Time, Day Shift (80 hours every 2 weeks) Location: Good Samaritan Hospital -252 S. 4th St. Lebanon, PA 17042 Job Description Under general supervision, coordinates all aspects of treatment, evaluation, support services and education for palliative patients. Coordinates clinical documentation from referring and treating physicians for each patient. Duties and Responsibilities Provides initial intake phone contact with palliative patients. Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment. Provides individualized information, support, and education regarding illness. Coordinates appointments for consultations and physician visits. Notifies primary care and specialty care providers when patient is admitted to the Hospital and/or significant changes in their condition. Follows patient via contact with inpatient nursing staff. Participates in interdisciplinary nursing rounds for inpatients (discharge planning process). Maintains two-way communication with providers offices in order to improve the process of coordinating care for palliative patients. Reports outcome of nursing/patient care/coordination activities to Referring Provider. Identifies patient and/or provider needs and develops quality programs to meet those needs. Ensures delivery of quality services for patients. Collaborates with outreach/education coordinators in the preparation and delivery of community and professional education programs. Protects patient confidentiality by promoting professional staff communications. Answer incoming calls and communicate with patients, care givers and palliative team. Coordinate prescription authorizations with pharmacies and patients insurance provider. Nonessential Accountabilities: Participates in interdisciplinary conferences (as needed). Responsible for the care and maintenance of department/entity equipment and supplies. Attends meetings as required and participates on committees as requested. Performs other related duties as identified. Demonstrates knowledge of the patients illness and clinical course over the life span of the patient. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. Qualifications Notes: Other combinations of formal education, training and experience may be considered. Required Experience: 2-3 years nursing experience Case Management, Palliative and/or Hospice experience preferred RN License required; BSN preferred Physical Demands: Mostly sedentary work Occasional prolonged standing/walking Occasionally lifts, positions, pushes and/or transfers patients Occasional reaching, stooping, bending, kneeling, crouching Skills: Proven outstanding interpersonal skills; demonstrated ability to function as a team player; excellent communication skills
