As part of achieving this goal, the Care Coordinator at VillageMD will follow up with patient s post-discharge to facilitate smooth transitions between levels of care and optimize patient outcomes, enabling our organization to avoid clinical setbacks resulting in readmission to the hospital.

Could this be you?

Integral to our Care Management team, the Care Coordinator will be accountable for ensuring appropriate utilization of resources and facilitating a positive experience for patients, families and caregivers following a Hospital or Skilled Nursing Facility stay. In addition, the Care Coordinator will collaborate closely with the care team to consistently communicate and document high risk patient profiles to provider and practice care teams. As a new member of our team, you ll work closely with our interdisciplinary care team to connect the dots of collaborative patient care.

How you can make a difference

  • Reaching out to patients that are discharged from the hospital and facilitating a smooth and safe transition back home
  • Coordinating patient care needs with acute care facilities, DME companies, our practice partners and other VillageMD staff
  • Ensuring patient needs are addressed immediately following discharge and communicating with the care team if clinical gaps are identified
  • Scheduling patients for follow up appointments with PCP
  • Request records from specialists enabling the continuity of care for both PCPs and members of the interdisciplinary care team
  • Assessing patients acuity and needs to initiate engagement for enrollment in Care Management
  • Support chronically stable patients as they transition from longitudinal Care Management to effective self-care and consistent, healthy behaviors
  • Detailing all interventions in EMR and care management platform both timely and accurately including all elements of the post-discharge plan

Skills for success

  • The ability to be flexible in an ambiguous and dynamic environment
  • A service orientation and a can do attitude
  • A willingness to learn on your own and take initiative
  • A low ego and humility; an ability to gain trust through good communication and doing what you say you will do

Experience to drive change

  • Licensed Practical Nurse licensure required
  • 3+ years as a Licensed Practical Nurse required
  • Must have at least three years of experience working in a clinical setting, primary care preferred
  • Comfort with technology including Microsoft suite of products
  • Utilizing a variety of electronic health records including data capture, data mining and reporting
