CARE MANAGER

Care Manager, RN - Inpatient Rehab

Description:

Performs the planning of care within the scope of practice and standards providing physician, other referral sources and post discharge resources with appropriate documentation demonstrating age-specific skill as required.

Provides a communication link with all disciplines and patients promoting the team approach to patient care by consulting with team members and leading patient care conferences.

Completes all required statistical information for patient admission and program improvement.

Maintains the medical record, team conference reports and case manager files to include subjective and objective findings, progress notes, payer and any non-treatment information.

Attends and participates in all designated hospital and rehab department meetings and in-services.

Maintains professional licensure, certifications, and competency requirements on a timely basis.

Performs and maintains all other duties/responsibilities as required.

Knowledge of medical terminology, disease process, psychosocial and home assessments, systems theory, and community organization.

Independently manages time constraints for providing all aspects of planning for patient care while patient is on the unit including discharge planning.

Demonstrates the ability to communicate with patients taking into consideration their age (pediatric, adolescent, adult, and geriatric). Counsels individual/groups of all ages with disabilities as needed.

Exhibits organizational skills, documentation and speech/writing skills, and public relation skills. Exhibits a professional, yet caring demeanor and appearance.

Possesses skills in case management: including interviewing, crisis intervention, counseling, and use of referral sources.

Exhibits ability to interface professionally with hospital staff and local/regional community agencies. Promotes a positive image of the Rehab Unit in the local community.

Must be able to adapt to change quickly, and to efficiently organize varied job responsibilities.

Possesses the ability to learn basic computer skills.

Education/Experience:

Graduate of an accredited School of Nursing, BSN preferred.

Experience in case management in acute or rehabilitation setting required.

One or more years clinical experience in acute care or rehabilitation setting preferred
