Case Manager II (Family Based Counseling)

Diakon Family Life Services offers a range of counseling and behavioral health services to children, teens, families and adult individuals. Services are provided through counseling centers in Mechanicsburg, New Bloomfield, Lewisburg, Montoursville and Williamsport as well as within several public-school buildings. Specialized programs include family-based mental health services, provided in the community and in-home, addiction counseling, trauma-focused therapy and SPIN, a service designed to assist children and youths who have problematic sexual behaviors.

General Responsibilities:

  • Responsible for serving as a resource person, case manager, counselor and trainer to support the Family Based Services Unit.
  • Responsible for providing in-home individual and family therapy to clients with social and emotional disturbances.
  • Creates systematic change through the use of a variety of therapeutic interventions with clients and their families.
  • Provides case management and crisis intervention as needed.
  • Childline & DHS FBI clearance results are required prior to beginning employment.


Education/Training: A Bachelors degree in social service or a related field of study required.

Experience: A minimum of three (3) years' experience in a related child welfare or human service position normally required.

ADA: Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills
