Clinical Pharmacist

The Anticoagulation Management Service (AMS) Pharmacist is responsible for providing comprehensive management of patients receiving anticoagulant therapy in the outpatient setting. The pharmacist collaborates with physicians, nurses, and other healthcare professionals to ensure safe, effective, and evidence-based anticoagulation care. This includes patient education, laboratory monitoring, dosage adjustments, documentation, and coordination of care in accordance with established clinical protocols and national guidelines. Must be a licensed pharmacist in the State of Massachusetts. Two years experience preferred.
