Job description Hide SUMMARY: The Clinical Pharmacist shall support the mission of MyCare Medical Group and the pharmacy department by preventing, identifying and resolving drug related problems to assure patient safety, improve patient outcomes and satisfaction and minimize costs. We consistently strive to reduce medication-related risk while documenting improved economic, clinical and patient outcomes. The MyCare Medical Group clinical pharmacist will provide support for 10-15 primary care clinic locations via population health management reports and strategies, individual patient contact and clinic level education as appropriate. The pharmacist will collaborate with the patient’s primary care provider to optimize evidence-based pharmaceutical care, develop appropriate care plans, and follow up with patients on individualized patient needs. Approximately 40% of your time will be spent in population health management working remotely. The remainder of your time will be spent onsite in clinics or meetings providing education to both clinic teams and patients. QUALIFICATIONS:Extensive knowledge of pharmacy and its related subdisciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of pharmacy practice.Ability to integrate with the healthcare team and work collaboratively to establish and maintain effective working relationships with other healthcare professionals, practitioners, patients, and familiesExperience in the medication reconciliation processWorking knowledge of CMS HEDIS measures, especially as they relate to Part C and Part DProficient at working with or learning how to work with Electronic Health Record (HER) systemsWorking knowledge of the use of enterprise data warehouses and patient registriesUnderstanding of the principles of clinical integration and population health managementEffective oral, written communication, and telephone skills.Good organizational skills to handle multiple priorities while remaining professional and calm.Ability to work with many diverse people, including elderly, disabled, mentally challenged, low literacy, impoverished, and underserved populations.Ability to work independently and perform functions with minimal supervision.Ability to multi-task and prioritize responsibilities and objectives. GENERAL DUTIES AND RESPONSIBILITIES:Review patients’ medications profile and make recommendations to enhance patient outcomes or safety as needed.Use professional judgment to provide timely, accurate and appropriate pharmaceutical care.Responsible for working with patient primary care provider and care management team to manage chronic conditions (diabetes, hypertension, COPD, etc.) and getting these patients to goal.Help prevent avoidable medication-related hospitalizations.Serve as a resource to practice physicians, nurses, and other team members on general drug information.Document clinical activities, outcomes, and quality intervention results.Participate in problem-solving where appropriate about best practice development for medications such as high cost, or otherwise sub-optimal therapies.Assist Director, Pharmacy Services in establishing and maintaining internal formulary and protocols and local education of practitioners.Consistently provide pharmaceutical care to patients to ensure appropriate prescribing, route, method of administration, and length of therapy.Monitor for age-appropriate response to drug therapy and for any adverse events from drug therapy.Exhibit competence in pharmacy clinical functions and accessing drug/patient information resources including lab values.Ensure compliance with state and federal laws and applicable rules and regulations for clinical pharmacy practice.Maintain professional and technical knowledge, licensures and certifications through continuing educationSupervise two technician roles managing prescription refill protocols and responses as well as medication adherence strategies.Accept and perform other duties as assigned.Maintains and promotes HIPAA compliance.Effectively communicates MyCare’s mission and core values.
