The Behavioral Health Discharge Planner provides discharge planning, financial counseling, psychosocial assessments, and limited emotional support for patients/families as indicated. Remains knowledgeable of community resources and refers patients appropriately to community agencies and post-acute providers. The discharge planner provides these services to adult and geriatric patients in an inpatient setting

Duties & Responsibilities

  • Creates a process for communicating and collaborating with members of the inter-disciplinary team involved in the care of patients.
  • Participates with the unit team in daily rounds with the patients and reviews assessments, sets goals, and prioritizes assignments.
  • Screens patients admitted and assesses those patients at high risk for post discharge needs.
  • Serves as a liaison for patients/families, nursing staff, and physicians.
  • They are the vital link for community, health, and social agencies that have previously provided services for the patient or will be providing services post discharge.
  • Provides the Medicare Important Message to patients prior to discharge as appropriate. • Educates the patient and family on the discharge planning process and facilitates patient flow through the healthcare continuum.
  • Arranging services in other medical facilities, other specialized institutions, or community agencies.
  • Assists the patient and/or family in applying for financial assistance, services, or counseling to the appropriate community agency or other source of help.
  • Validates payer coverage and assures appropriate agencies are utilized.
  • Maintains expertise of available resources and federal and state medical assistance programs.
  • Screens for abuse, neglect, or exploitation and refer appropriately to state or community agencies in accordance with state and federal regulations.
  • Collaborate with team members to identify and validate avoidable days.
  • Maintains professional integrity and confidentiality of information in accordance with AnMed policies and procedures, state and federal requirements, and applicable laws.
  • Adapts quickly to changes in criteria and/or focus and fulfills duties responsibly with little supervision.
  • Able to work with variable patient populations.
  • Stays abreast of departmental and organizational changes.
  • Recommends process improvements of policies and procedures as necessary.
  • Performs other duties as assigned.

Qualifications

  • Bachelor's degree in social sciences or a four-year degree with related experience.
  • Knowledgeable regarding community resources.
  • Excellent communication skills.
  • Works in collaborative team approach.

Preferred Qualifications

  • Two years of hospital discharge planning.
  • One to two years of experience in social services or patient care experience.
