Entry to Care Nurse Practitioner

JOB SUMMARY:

The Entry to Care Nurse Practitioner will serve in a clinical capacity for direct patient care.

This role is not intended to provide primary care continuity and will not carry a patient panel, but rather to streamline entry to care through exclusively telehealth assessment of past medical, social, surgical, and family history of new patients, entry/review of current medication lists for new patients, initiation of external records retrieval as applicable, as well as any acute time-sensitive referral needs prior to the patient's first in-person visit. Additional acute and limited-concern visits may originate from other internal resources as scheduling allows and acute patient needs arise. As visit attendance and time permits, additional EMR desktop coverage may be assigned for providers who are out of office, and this will be evaluated on a rolling basis.

This position will require close communication with scheduling teams and clinical teams to coordinate and monitor new patient scheduling for follow-up with assigned PCP after completion of the telehealth intake visit. Additional communication with pharmacy, social work/case management, nursing, or other teams may be required as applicable to new patient need upon intake.

MAJOR DUTIES AND RESPONSIBILITIES:

Patient Focus
• Coordinates and ensures effective clinical appointment access and flow of care between departments and teams
• Effectively engages patients in care and offers coordinated services with other members of the internal care team.

Compliance, Policy, and Procedure
• Practices within established policy and protocol and adheres to best practice recommendations for clinical care as appropriate.
• Ensures all policies and procedures are in place and followed accordingly, and annual and periodic training and competencies are completed appropriately

Analytical and Critical Thinking
• Maintains the standard of clinical care, using diagnostic testing appropriately and providing appropriate treatment as applicable
• Considers the psychosocial aspects of both health and disease
• Uses diagnostic testing appropriately yet cost-effectively

Checking, Examining, and Recording
• Maintains complete and comprehensive documentation of all care provided, whether by phone, portal, or complete visit.
• Record retrieval of quality metrics, hospital records from prior 3 months, prior primary care.

Professionalism
• Seeks physician, specialist, or other colleague and inter-departmental consultation when appropriate
• Demonstrates professionalism and integrity in all aspects of patient care

SKILLS AND ABILITIES:
• Ability to work effectively and respectfully in a multidisciplinary, culturally diverse team
• Ability to build and maintain effective relationships
• Ability to coordinate, organize, and remain flexible
• Excellent technological familiarity with Microsoft products (Outlook, Teams, etc.)
• Knowledge of hormone treatment and primary care needs of transgender people
• Knowledge of primary care as well as sexually transmitted infections, trauma-based care, and social determinants of health

EDUCATION AND/OR EXPERIENCE:
• Required: Active and unrestricted license to practice medicine in Maryland
• Required: Completion of an accredited NP training program and board certification
• Experience providing LGBTQ health care preferred, including gender-affirming care.
• Experience in HIV care, general adult medicine, and general pediatric medicine preferred.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Work is anticipated to be entirely remote/via telehealth. Occasional on-site presence may be required for training, meetings, or other needs arise, and will be coordinated with staff in advance whenever possible. Ability to perform functions normally completed during the course of clinical care (i.e., sitting, standing, fine motor coordination), use of the keyboard for typing or editing of EMR notes, clinically evaluating and communicating plans to patients.
