Type of Opportunity: Per Required Need FTE: 0. 001000 Exempt: No Work Schedule: 12 Hour Days

Summary:Provides skilled aide care to the patient under the supervision and direction of the primary registered nurse or clinician and under an aide care plan as ordered in conjunction with the physician


Responsibilities:Perform personal care of patient including bathing, care of mouth, skin and hair, following all policies and procedures. *Assist patient with bed and commode transfers, ambulation with or without equipment, including hoyer lift, under the supervision of physical/occupational therapist or registered nurse.*Assist patient with prescribed exercises that have been taught by appropriate agency professional personnel.*Assist patients by handing bottle or reminding patient to take oral medications which are labeled in a container, with dose and time already set up; by patient, family or nurse. *Monitors vital signs, skin, bowel and bladder function and reports to the primary RN/clinician any significant problems or changes in patient status as ordered on the aide care plan.*Participates with other team members in treatment care plan, offering suggestions for modification.*Follows written treatment care plan as established by the team. Requests assistance and instruction with new or unfamiliar procedures or situations.*Reinforces teaching of personal care techniques including care of skin, repositioning and transferring with patients/family following training by R.N. or therapist. *Provide emotional support.*Demonstrates behaviors conducive to creation of a productive, functioning team.*Documents care according to agency policy, regulation and standards. *Provide simple meal preparation, cleaning, laundry and respite care as ordered following instruction by clinician.*Provide simple wound care and bowel care per Aide plan of care*Maintains productivity within agency standards.*Participates in agency continuous quality Improvement activities as required.*May be assigned to the Home Health Aide Float Pool. Staff is then required to comply with all the Home Health Aide Float Pool Guidelines.*Performs other functions as required


Other information:High School diploma/GED preferred, CNA certification required for Albuquerque agency, regional CNA certification required within 18 months of hire. Less than one year of recent direct patient care experience. Education, Demonstration and testing of competencies are required according to the Medicare Conditions of Participationif in Home Health
