Job Description
A clinic in Delaware is seeking a Hospice Physician to provide quality patient care for a Locum Tenens assignment.
Assignment
- Dates: ongoing- open to discussing a timeframe (currently looking for perm providers)
- Schedule: ~24 hours a week (Tuesdays are a must work day, or at least able to attend weekly interdisciplinary group meetings)
- days and hours are flexible: providers will coordinate with the scheduler on which days they can work
- Setting: Inpatient
- Call: after hours, phone only
- Volume: In home visits, assisted living/nursing home visits, and some hospitals- volume varies based on what providers tell scheduler they can work, could see 5-8 patients face to face when doing field visits but will depend on their preferred hours
- Patient Ages: Geriatrics
- Procedures: none
- Cases: Patients in the last stages of incurable;e diseases, or general last stages of life- can be chronically ill with Cancer, COPD, Diabetes, Heart Failure, etc.; Providing care and support for patients and families
- - Assessing patients for new hospice eligibility, and consulting on a long-term health plan.
- - A combination of in-home visits, Nursing homes/ Assisted living facilities, and hospitals- visiting patients on hospice and making sure they are comfortable in the later stages of their lives and they are following their plan of care
- Support Staff: NPs, RNs, CNAs, Social workers, etc. When in-home, likely to be solo, assisted living/nursing homes will have RNs, CNA, etc onsite. Hospitals have full hospital staff onsite
- EMR/Charting: Homecare Homebase
Requirements
- Boards: Board Certified; Board Eligible
- Certifications: ACLS; BLS; DEA
