We are growing and adding positions to our team! The job duties for the position of the *Hospice LVN *are listed below.

*Job Summary:*

The LVN is primarily responsible for administering skilled nursing care, under the supervision of a registered nurse, for patients/clients of all ages in their places of residence. Participates in the coordination of coordinate care with the interdisciplinary team, patient/client, family and referring agency.

*Duties/Responsibilities:*

· Works in patients’/clients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather

· Under the direction of the RN, assists in identifying the patient’s/client’s physical, psychosocial, and environmental needs as evidenced by documentation.

· Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.

· Provides patient/client and caregiver education.

· Provides care using infection control measures that protect both staff and patient/client.

· Assures the continuity of care through delivery of quality patient/client care.

· Provides effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner

· Monitors assigned cases to ensure compliance with requirements of third-party payor.

· Demonstrates commitment, professional growth, and competency.

· Promotes Agency philosophy and administrative policies to ensure quality of care.

*Required Skills*

· Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.

· Proof of current CPR certification

*Job Qualifications (Education, Experience, Licensure)*

· Graduate of an accredited school of vocational nursing.

· Licensure: Active state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.

· Current valid driver’s license.

· One year of experience as a licensed vocational/practical nurse in a clinical care setting required. Home health experience, preferred.

· Reliable transportation and valid auto liability insurance.

*Physical Requirements:*

· Prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients.

· Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition; meet patient/client and family individualized psychosocial needs.

· Requires hand-eye coordination and manual dexterity.

Pay: $25.00 - $28.00 per hour
