Position Overview

The Intake Caseworker is responsible for the coordination and completion of all tasks, documentation and communication relevant to the provision of intake management and support services for clients beginning with pre-admission and ending with discharge.

Key Responsibilities

  • Continually model and uphold the high standards of quality customer satisfaction. The Intake Caseworker position is a highly visible "ambassador" role, which requires the establishment and maintenance of positive rapport with parents and customers.
  • Process all referral calls in a timely fashion to ensure the shortest period of time between referral call and assessment.
  • Coordinate with the referral's source and/or family a mutually agreeable date and time for the Intake Assessment.
  • Coordinate with all internal and external customers regarding admission to the programs and provide program tours, when necessary.
  • Maintain client lists and track and prepare referral and other management reports, if applicable.
  • ordinate Medicaid services received by the child in the community and transition the child to home and community-based living with outpatient treatment as part of discharge planning.
  • Facilitate the exchange of health information and documentation of care received by the child in the community, including attending any community provided care.
  • Must coordinate care if child attends community school and delivers services through the provider.
  • All other duties as assigned by management.

Qualifications

  • Bachelor's Degree in Human Services related field preferred or equivalent combination of education and experience to perform the essential functions of the position.
  • Bilingual Spanish/English preferred.
  • Experience with acute emotionally challenged children or adolescents preferred.
  • Valid driver's license required.
  • Must be 21 years of age.
