Summary Job Summary: Provides outpatient supervisory and clinical services as part of AHS’s Integrated Behavioral Health model in the primary care and specialty care areas; supervises, trains, and evaluates the work of outpatient social work staff; in partnership with Ambulatory leadership, implements and evaluates Integrated Behavioral Health model into ambulatory services; provides direct client service. DUTIES & ESSENTIAL JOB FUNCTIONS : NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification. 1. Directs and reviews the work of outpatient social workers; assists in setting work standards and ensures that social work professional standards are maintained. 2. Guides workers in diagnosing and developing treatment strategies and with challenging client situations; evaluates performance. 3. Provides clinical services to population of vulnerable, low-income patients as part of Integrated Behavioral Health team. 4. Conducts initial interviews, assess, and provides brief clinical interventions and referrals. 5. Performs individual and group psychotherapy when appropriate. 6. Identifies and pursues necessary resources for patient care; identifies and removes barriers for staff to promote service delivery 7. Conducts mental status examinations and formulates DSM V related diagnoses. 8. Provides training and ongoing consultation for clinic providers, clinic staff, behavioral health staff, and students; provides education on specific topics as requested. 9. Represents the patient’s needs and rights to providers and community in a variety of settings. 10. Provides crisis intervention as needed. 11. Maintains appropriate charting - document progress notes and treatment plans as required by the clinic protocol and other mandated regulatory agencies; prepares correspondence regarding social evaluation of patient and patient follow-up 12. Tracks outcomes, maintains patient registries, and targets interventions based on quality metrics and behavioral health best practices. 13. Leads planning meetings, case conferences, and care team huddles; works collaboratively with interdisciplinary team. 14. Confers with Ambulatory and Behavioral Health leadership on specific case problems and; confers with hospital administrators on specific case problems. 15. Develops, implements and evaluates orientation and training programs for ambulatory social work staff. 16. Assists with implementation of standard procedures and interventions for Integrated Behavioral Health model in ambulatory care. 17. Secure accurate social and personal data and record such data systematically. 18. Performs other duties as required. Qualifications: Required Education: Master’s degree in Social Work awarded by a recognized college or university. Preferred Education: N/A Required Experience: Three years post-MSW experience with mental health and substance use counseling; one year of experience in a lead or supervisory capacity. Preferred Experience: N/A Licenses/Certifications: Licensed in the State of California as a Clinical Social Worker (LCSW). Additional Information Qualifications: Required Education: Master’s degree in Social Work awarded by a recognized college or university.
