Position Title: Licensed Practical Nurse

LPN- Shift hours: 8A-4P,  4P-12P,  12P-8A

(Every Other Weekend)

Position Overview:

Seeking compassionate and skilled Licensed Practical Nurses to provide hands-on nursing care in a long-term care setting. LPNs will work under the supervision of a Charge Nurse and may supervise Nursing Assistants or aides.

Key Responsibilities:

• Monitor and report changes in residents' conditions
• Administer medications (excluding IV) and observe for side effects
• Document care and update care plans as needed
• Participate in interdisciplinary care planning and team discussions
• Maintain accurate nursing records and ensure compliance with facility protocols
• Provide support during resident transitions and orientations
• Attend staff meetings and ongoing training sessions

Requirements:

• NYS LPN License or limited permit (must obtain license within 6 months)
• Completion of an approved practical nursing program
• Ability to operate standard medical equipment
• Strong observation, communication, and documentation skills
• DL or SSN required at time of submission
• Must complete 3-day orientation prior to starting
