Job Title: Licensed Practical Nurse

Location: Elmira, NY 14901

Duration: 13 weeks to long-term 

Shift: 06:45 AM – 03:15 PM and 02:45 PM – 11:15 PM

Job Responsibilities:

• Provide direct patient care and monitor residents' health and well-being.
• Administer medications in accordance with physician orders, facility policies, and applicable regulations.
• Document all nursing care, medication administration, and changes in residents' conditions accurately and promptly.
• Follow established facility policies and procedures, nursing best practices, and the New York State (NYS) nursing Code of Ethics.
• Monitor residents for changes in health status and promptly report concerns to the RN Supervisor or appropriate healthcare provider.
• Assist with coordinating and managing daily unit operations to ensure quality patient care.
• Provide guidance and direction to nursing aides to support safe and effective care delivery.
• Monitor nursing aides' break schedules and report any concerns or staffing issues to the RN Supervisor.
• Collaborate with RNs, nursing aides, and other healthcare professionals to maintain continuity of care.
• Maintain compliance with infection control protocols, patient safety standards, and applicable state and facility regulations.
• Demonstrate leadership, professionalism, accountability, and effective communication in the performance of nursing duties.
• Ensure residents' rights, dignity, privacy, and confidentiality are maintained at all times.

Documents Required:

• Active LPN License
• BLS/CPR Certification
• Covid-19 Vaccination card.
