Required Qualifications
Licensed Practical Nurse, Home Health
Licenses: LPN(Compact Accepted)
Certifications: BLS-AHA(Copy needed)
SSN Required
DOB Required
DL, AHA BLS, and Completed Franciscan MVR report, copy of valid driver’s license and proof of full coverage auto insurance with required limits as outlined: $100,000 each person/ $300,000 each accident bodily injury / $100,000 property damage OR $300,000 combined single limit.
• Years of Experience: 3 years of acute care or LTC AND 1 year of HH experienceWork History: Should not be more than 0 day of gap in
Drivers license
Personal Auto insurance (full coverage)
