Licensed Practical Nurse (LN IID)
Location: Willis Knighton North
Department: Discharge Lobby
Schedule: Monday through Friday, 8-hour shifts
Position Summary:
The Licensed Practical Nurse (LPN) in the Discharge Lobby plays a vital role in ensuring patients leave the hospital with clarity, confidence, and compassion. This position focuses heavily on discharge education, coordination of care, and creating a positive final experience for patients and families. The ideal candidate is detail-oriented, patient-focused, and brings a high level of professionalism and kindness to every interaction.
Key Responsibilities:
- Provide clear, thorough discharge education to patients and families, including medications, follow-up appointments, and care instructions
- Reinforce physician and RN discharge plans to ensure patient understanding and compliance
- Collaborate with nurses, providers, case management, and support staff to facilitate timely and safe discharges
- Answer patient questions and address concerns with patience and empathy
- Ensure all discharge documentation is complete, accurate, and compliant
- Assist with coordination of transportation and post-discharge needs as appropriate
- Maintain a welcoming, calm, and organized discharge environment
- Promote a culture of teamwork, respect, and excellent customer service
Qualifications:
- Current Louisiana LPN license in good standing
- Previous acute care or med-surg experience preferred
- Strong communication and patient education skills
- Ability to multitask and manage patient flow efficiently
- Positive attitude and team-oriented mindset
- Must demonstrate professionalism, compassion, and the ability to work well with all team members
