Overview:
Our patient-focused healthcare team in Philadelphia is seeking a Licensed Practical Nurse (LPN) to provide compassionate, hands-on care. This full-time role offers a structured weekday schedule and the opportunity to build strong patient relationships while maintaining a healthy work-life balance.

Key Responsibilities:

• Administer medications, injections, and vaccinations in accordance with clinical protocols
• Monitor patient conditions, vital signs, and responses to treatment
• Assist with patient intake, screenings, and routine clinical support
• Perform basic wound care and assist with specimen collection
• Support patient education and chronic disease management initiatives
• Maintain accurate and timely documentation within electronic medical records
• Respond to urgent patient care situations, collaborating with providers and interdisciplinary teams to ensure quality outcomes
• Follow infection control, safety, and regulatory compliance standards

Qualifications:

• Active and unrestricted LPN license in Pennsylvania
• Current BLS certification or ability to obtain upon hire
• Previous LPN experience in a healthcare or clinical setting preferred, ideally in correctional health, urgent care, behavioral health, or primary care
• Strong communication, organizational, and patient care skills, with EHR proficiency preferred
• Ability to thrive in a fast-paced, team-oriented healthcare environment
