Call Requirements: All work every third weekend
Floating policy: All Social workers float-except trauma.
Must have acute care adult hospital case management
MIDAS/Meditech Experience, PASRR, Navihealth
MSW or LCSW
1 year experience
FL lic required
Patient Experience requirements: Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT
Documentation System: MIDAS/Meditech, Nate, Navihealth, Care Team Assignment
List Typical Procedures Performed on Unit(s):
Discharge planning, MDR, Referrals, Working with Family and Patients
