Locums Nurse Practitioner or Physician Assistant
– Home Care Visits

We are seeking a Locums Nurse Practitioner (NP)
or Physician Assistant (PA) for a flexible position split between Brooklyn and Flushing, NY. The role involves providing comprehensive primary care to
geriatric PACE participants in their homes, ensuring high-quality,
patient-centered care.

• Small daily patient census: 3-5 patients
  per day
• Flexible schedule: 4 days per
  week, with two days in Amityville and two days in Flushing
• No nights, no weekends 

Compensation: $75-80 per hour

Requirements:

• Active NP or PA Board Certification
• Medicare & Medicaid Provider ID
  (REQUIRED)
• Minimum of 2 years of experience preferred
• Spanish speaking is preferred, but not
  required
• Experience in home care and geriatrics
  preferred 

Amityville and Flushing are both located in New
York, but they offer different experiences and settings.

Amityville, located on the south shore of Long Island,
offers a suburban feel with easy access to beaches, parks, and local amenities.

Flushing, is a vibrant and diverse neighborhood in the
borough of Queens, New York City. Known for its rich cultural scene, Flushing
boasts a large Asian community and is renowned for its excellent dining,
shopping, and cultural attractions.
