Responsibilities

  • Provide direct patient care including monitoring vital signs, administering medications, and assisting with activities of daily living (ADLs).
  • Perform basic nursing procedures such as wound care, catheter care, and specimen collection per facility policy.
  • Document patient assessments, interventions, and responses accurately in the medical record/EHR.
  • Collaborate with RNs, physicians, and interdisciplinary team members to coordinate patient care and report changes in condition.
  • Educate patients and families on care plans, medications, and follow-up instructions.
  • Maintain infection control, safety standards, and comply with facility and regulatory policies.

Required Skills

  • Valid LPN/LVN license in the state of practice.
  • Current BLS/CPR certification.
  • Competency in medication administration and basic nursing assessments.
  • Familiarity with electronic health records (EHR) and accurate charting.
  • Strong communication, critical thinking, and time-management skills.
  • Ability to work flexible shifts, including nights, weekends, and holidays as required.
  • Team-oriented attitude and ability to adapt to different clinical settings.

Qualifications

  • Valid LPN/LVN license in good standing.
  • Minimum of 1 year of clinical nursing experience preferred; specialty experience (e.g., long-term care, med-surg, clinics) a plus.
  • Current BLS certification; additional certifications as required by assignment.
  • Ability to pass background check, drug screen, and provide up-to-date immunization records/TB testing.
  • Reliable transportation and flexibility to travel to assignment locations when necessary.
