LPN Home Health Travel Contract – Lawrenceville, GA
Location: Lawrenceville, GA 30046
Position: LPN – Home Health
Contract Length: 13 Weeks
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Pay: $1,700–$1,800/week
We're seeking an experienced Home Health LPN for a 13-week travel contract in Lawrenceville, GA. This is an excellent opportunity to join a respected home health team providing compassionate, high-quality patient care in home and facility settings.
Responsibilities
Provide skilled nursing care in accordance with physician orders and agency policies.
Perform Start of Care visits, wound care, follow-up visits, and other skilled nursing services.
Coordinate patient care using the nursing process.
Supervise and evaluate Home Health Aides and other LPN/LVNs as needed.
Complete accurate and timely documentation using HomeCare HomeBase (HCHB).
Schedule & Productivity
Monday–Friday, 8:00 AM – 5:00 PM
One weekend of on-call coverage per month
Average of 5–8 patient visits per day
Productivity expectation: 30 points/week (approximately 6–8 points per day)
Requirements
Active Georgia LPN License (license must be in hand)
Minimum 1 year of recent Adult Home Health experience (or 1 year of Hospice experience if applicable)
Previous Home Health experience is required
Current BLS/CPR Certification
Proficiency with HomeCare HomeBase (HCHB) within the last 6 months (retraining available if experience is older)
Reliable transportation, valid driver's license, and required auto liability insurance
Willingness to travel up to 75 miles from the home clinic
Travel & Mileage
Mileage reimbursed at the IRS standard rate between patient homes
Average travel: approximately 30–40 miles per day
Travel to the first patient and home from the last patient is not reimbursed
Additional Information
Local candidates welcome (same pay rate)
Guaranteed hours are not available due to patient census; floating opportunities may be offered if needed
Dress Code: Scrubs
EMR: HomeCare HomeBase (HCHB) and OASIS familiarity preferred
Opportunity for permanent placement may be available after the contract
