Job Title: LPN / LVN – Long Term Care
Location: Utica, NY
Facility: Mohawk valley - rehabilitation and nursing center (rnc)
Hourly pay rate: $32 on W2 / $34 on1099
Contract Length: 13 Weeks
Work Week: Sunday – Saturday
Shift: 8‑hour evenings, 2:30 PM – 10:30 PM
Hours: 37.5 guaranteed per week, OT after 48 hours (daily OT after 24 hours based on multiplier)
Radius Rule: 75 miles – locals accepted at local rate
Candidate Type: Local or Travel
Requirements:
- Active NY LPN license
- BLS certification (AHA)
- Minimum 1 year of LTC experience
- Must float to all units within the building
- Every other weekend required
Facility Details:
- 40‑bed unit
- Patient populations: MS, orthopedics, dementia, memory care
- EMR: PointClick Care
Required Documents:
- Updated Resume
- Active LPN Licence (NY Licence mandatory)
- BLS Certification
- COVID-19 Vaccination Card
- Valid Driver’s licence
- Three professional references (from past managers/supervisors within the last 2 years)
- Signed Criminal Background Consent Form
- One year of Long-Term Care experience needed.
