Program and Job Responsibilities

Job responsibilities include:

• link clients to community resources and social support systems
• Virtual visits can be used to assist with duties
• Support clients in following individualized care plans and identifying case management goals
• Complete documentation on services provided in EHR system

Required Qualifications

Education

• Bachelor degree in the human/social services field. (sociology, child development, social work, psychology, primary education)

Licenses/Certifications

• Driving positions require a current Driver s License, must be 21

Preferred Qualifications

• One year of behavioral health experience
