They will be seeing 2-4 patients a day when in the field. 2. This is a full-time, home health-based position with travel reimbursements provided. 3. Field/Territory: To Be Assigned within 2 Hour Driving Radius Additional Job Details: 1. Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first. 2. Conducts assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur in-home.) 3. Prescribe medications, order tests, and collaborate with patient's Monogram physician. 4. Documents items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan. 5. Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning. 6. Complete all documentation and paperwork in a timely manner. 7. Maintains quality of care and productivity standards as defined by the practice. 8.
