Position Overview

We are seeking a compassionate, motivated Nurse Practitioner (NP) to join our growing clinical team. This provider will play a critical role in delivering high-quality primary care services in a rural setting while helping shape an innovative model of care for vulnerable populations.

This is an opportunity to step into a foundational role within a new clinic with strong leadership support, clinical autonomy, and the ability to make a meaningful impact in the community.

Key Responsibilities

  • Provide comprehensive primary care services across the lifespan
  • Conduct physical exams, assessments, and diagnostic evaluations
  • Diagnose and treat acute and chronic conditions
  • Prescribe medications and manage treatment plans
  • Provide preventative care, screenings, and patient education
  • Collaborate with interdisciplinary teams including behavioral health and case management
  • Maintain accurate and timely documentation in the EMR
  • Ensure compliance with all state, federal, and Rural Health Clinic regulations
  • Participate in quality improvement and care coordination initiatives

Requirements

Qualifications

  • Active and unrestricted Kentucky APRN license
  • Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP
  • DEA license (or ability to obtain)
  • Experience in primary care, rural health, or underserved populations preferred
  • Experience with individuals with I/DD is a strong plus
  • Strong clinical judgment and commitment to patient-centered care
