Responsibilities

What You'll Do

  • Provide comprehensive outpatient primary care to patients across the lifespan.
  • Conduct physical examinations, assessments, diagnosis, and treatment planning.
  • Develop and implement individualized treatment plans based on patient needs.
  • Manage acute and chronic medical conditions and provide preventive care.
  • Educate patients and families on health promotion, disease prevention, and treatment plans.
  • Coordinate care with specialists, community resources, and other members of the healthcare team.
  • Maintain timely, accurate, and thorough clinical documentation.
  • Collaborate with an interdisciplinary team to promote positive patient outcomes.

Required Qualifications

  • Graduate of an accredited Nurse Practitioner program.
  • Current New Mexico licensure as a Family Nurse Practitioner (FNP).
  • Board certification.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to work collaboratively with patients, families, and multidisciplinary teams.
  • Commitment to providing compassionate, culturally responsive, patient-centered care.

Preferred Qualifications

  • Experience providing outpatient primary care.
  • Experience caring for patients across the lifespan.
  • Bilingual English/Spanish or English/Navajo skills.
  • Experience working with diverse populations and community-based healthcare.
  • Commitment to serving rural and underserved communities.
