Job DescriptionApply now and join our Atlas team! JOB DESCRIPTION Nurse Practitioner JOB SUMMARY: Internal Medicine Clinic seeking a full time Nurse Practitioner. Provider must be able to evaluate and treat patients, have a caring and professional beside manner, must be able to collaborate with other providers, and work independently as well as with other staff members. Clinic hours are Monday - Thursday 7:30am - 6pm. Location: Wasilla, AK Pay: $110,000 - $125,000 (depending on experience) QUALIFICATIONS: Education, Preparation and/or experience to include: * Possesses a National Board Certification as a Nurse Practitioner * Current license as a Nurse Practitioner in the State of Alaska * Minimum of one (1) year experience in an acute care setting or equivalent experience * Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair as well as a good driving record * Current CPR certification * Strong communication and interpersonal skills RESPONSIBILITES: * Practices nursing with respect for individual, cultural, and spiritual differences * Maintains confidentiality in all aspects of the job * Communicates information using Ancora's Hospice processes and technology * Performs other duties as assigned * Demonstrates competency in areas of assignment or identifies the need for additional training * Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice: * Assessment skills as applied to the client, family/support system, and environment * Teaching skills according to the client's needs and within the context of the client's environment, value system, and physical/emotional status * Medication administrations * Provides the Face to Face for Hospice Patients. * Manages client's discomfort and provides symptom relief * Consults with and educates the patient/family regarding: * The disease process * Self-care techniques * End-of-life care * The process for dealing with issues of ethical concern * Participates in the preparation of patient transfer, discharge plan and discharge summary * Provides services and/or treatments requiring substantial and specialized nursing skills related to palliative and end-of life care * Initiates appropriate preventative and rehabilitative nursing procedures * Initiates referrals as needed to other health team members when delegated by the Case Manager * Incorporates analysis of data and current scientific findings into patient care * Performs initial comprehensive assessments and ongoing assessments of the impact of the terminal diagnosis on the patient's physical, functional, nutritional, psychosocial and environmental needs and activities of daily living * Assessments include at a minimum the following: * Risk for pathological grief * Cultural and spiritual implications * Verbal and non-verbal communication patterns * Determines the scope and frequency of services needed based on acuity and patient/family needs * Continually evaluates and assesses the provision of hospice care and the patient/family's response * Assesses the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care * Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility * Ensures the development of an initial plan of care with the input of the Interdisciplinary Team * Coordinates the implementation of the plan of care for each patient and recommends revisions to the plan as necessary * Ensures that the plan of care is implemented with a focus on palliative care * Documents legibly and according to Agency documentation guidelines and standards. * Follows Agency guidelines for completion of forms, reflecting the orders and care plan * Prepares Clinical and Progress notes that demonstrate progress toward established goal(s) * Ensures that finding and ongoing services are documented in the medical record for each patient contact * Documents verbal orders received from the physician in the medical record * Coordinates all patient/family services and prioritizes needs with the members of the interdisciplinary team * Assigns written patient care duties to individual Hospice Aides * When appropriate, assigns a licensed practical nurse to provide nursing services in accordance with Aegis policies * Informs the physician, Medical Director and other IDT team members of changes in the client's needs and outcomes of intervention * Ensures the communication between Ancora Hospice and other health care providers * Evaluates own needs for support and uses identified system(s) to meet those needs * Supervises LPN/LVNs, Hospice Aides, Paraprofessionals and other team members as delegated by the RN Case Manager * Provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate care * Conducts supervisory visits to determine the quality of care being given b
