Full-time Acute Care Nurse Practitioner or Physician Assistant – Inpatient Geriatrics

HHCMG Geriatrics is seeking an experienced Acute Care Nurse Practitioner (ACNP) or Physician’s Assistant (PA) for their growing inpatient geriatric consult program at Hartford Hospital. You will work with a team that consists of Geriatricians, home health care providers, social workers, case managers, and resource coordinators that will be focused on managing care.

Hartford Healthcare Medical Group (HHCMG) is growing our Geriatrics program. Built on an institute model, our program currently provides Geriatric care, consultation and expertise to geriatric patients in various acute care, ambulatory and facility settings.

Position details:

  • Full-time, weekdays only

  • Location: Hartford Hospital

  • On call as per division policy

Responsibilities include:

  • Diagnose, prescribe and treat within the scope of the Acute Care Nurse Practitioner/Physician Assistant and consistent with standards of care

  • Manage patient care on a longitudinal basis, with the primary objective of delivering proactive chronic disease care and significantly reducing avoidable hospitalizations

  • Partner with the patient’s primary care provider and specialists to deliver integrated care

  • Instructs patients/caregivers on proper medication and treatment regimens

  • Communicates with patients, caregivers, agency nurses, other providers as necessary to ensure proper diagnosis and treatment

Requirements:

  • Trained as an Acute Care Nurse Practitioner or Physician Assistant

  • Experience and a passion for providing patient-centered care to a geriatric patient population focused on quality, prevention and education

  • A willingness to work closely and collaboratively within an interdisciplinary team

  • An unrestricted State of Connecticut Medical License (or eligible)

Regular Standard Hours Per Week: 40

Schedule: Full-time (40 hours)
