Qualifications

License & Certifications

Registered Nurse license issued by the state in which the caregiver practices, and

Advanced Practice Nurse Prescriber license issued by the state of Wisconsin, and

Nurse Practitioner (AANP) certification issued by the American Academy of Nurse Practitioners (AANP), or

Nurse Practitioner (ANCC) certification issued by the American Nurses Credentialing Center (ANCC), and

Adult Psychiatric & Mental Health Nurse Practitioner (PMHNP-BC) certification issued by the American Nurses Credentialing Center (ANCC), and

Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA) Registration, and

Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). May obtain within 6 months based on department discretion.

Education

Master's Degree in Nursing.

Experience

Typically requires 2 years of experience in providing and coordinating care for patients with acute, critical and chronic health conditions.

Maintains practice within both an outpatient clinic and inpatient hospital setting performing complex care coordination and advanced care to patients with complex, acute, critical and chronic health conditions.Demonstrated high level of clinical proficiency, and excellent decision-making skills.Demonstrated ability to work independently and as an effective member of a health care team.Excellent oral and written communication skills.  Ability to effectively collaborate with and establish/build relationships with others.Proven organizational skills and ability to prioritize effectively.
