SUMMARY: The Nurse Practitioner is a licensed practitioner who is responsible for managing health problems and coordinating health care of the sub-acute and nursing facility patient in accordance with state and federal rules and regulations and the nursing standards of care. Appropriate clinical management is conducted in collaboration with other healthcare team members and physicians.
EDUCATION, QUALIFICATIONS, CREDENTIALS:
* Graduate of an accredited Nurse Practitioner Program
* Current and/or pending Professional Licensure
* Current Nurse Practitioner certification in the specialty area
* Current provider card in Basic Life Support (BLS)
* Minimum of one (1) year of experience as a Nurse Practitioner; experience in sub-acute care and/or geriatrics preferred, but not required

The Nurse Practitioner must be able to perform each essential function effectively to be successful in this position:
* Functions independently to perform age-appropriate history and physicals for complex acute and chronically ill patients in the sub-acute and nursing facility settings.
* Orders and interprets diagnostic and therapeutic tests relative to the patient's age and disease-specific needs.
* Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
* Implements interventions to support the patient to regain or maintain physiologic stability.
* Monitors the effectiveness of interventions and updates the plan of care and treatment as needed.
* Facilitation the patient's transition within and between health care setting, e.g., admitting, transferring, and discharging patients.
* Collaborates with multidisciplinary team members by making appropriate referrals.
* Facilitates staff, patient, and family decision-making by educational tools.
