Nurse Practitioner / FNP

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on preventive health measures, chronic disease management, and acute care. The FNP will work collaboratively with physicians and other healthcare professionals to deliver high-quality patient-centered care in a supportive environment.

• Family Medicine (ages and specialty vary by health center) Independent family practice experience including routine wellness care, chronic care management, and urgent visit, preferred
• Primary Care, Acute Care
• Must be comfortable seeing ages 2+

Minimum Job Requirements for Nurse Practitioner:

• Active license & current board certification required
• Independent practice provider preferred, where applicable per State regulation.
• CPR/BLS certification required at time of start date
• Independent family practice experience including routine wellness care, chronic care management, and urgent visit, preferred

Schedule:

Start Date: 4/1/2026

End Date: 5/29/2026

• Monday: 8:00–4:30 (0:30 lunch) – 8 hrs
• Tuesday: 9:00–5:30 (0:30 lunch) – 8 hrs
• Wednesday: 8:00–4:30 (0:30 lunch) – 8 hrs
• Thursday: 9:00–5:30 (0:30 lunch) – 8 hrs
• Friday: 8:00–4:30 (0:30 lunch) – 8 hrs
