SCOPE OF POSITION:
The Nurse Practitioner is responsible for providing general medical care and treatment to residents at Rockingham County Rehabilitation and Nursing Center under the direction of the Physician. This role operates within the scope of practice for Nurse Practitioners and in accordance with State and Federal regulations, as well as established standards of care. The Nurse Practitioner supports the delivery of safe, effective, and individualized resident care.

We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. The Nurse Practitioner plays a key role in this mission by assessing residents' health status, developing and managing plans of care, and evaluating outcomes to ensure appropriate treatment and quality care. The Nurse Practitioner orders and interprets diagnostic and therapeutic tests, prescribes pharmacologic and non-pharmacologic treatments, and monitors the effectiveness of interventions. They utilize the Brief Cognitive Assessment Tool (BCAT) to assess memory and executive function, supporting staff in determining appropriate care levels for residents with cognitive impairment. Working collaboratively with physicians and the interdisciplinary team, the Nurse Practitioner reviews case information, makes appropriate referrals, and supports residents through transitions including admissions, transfers, and discharges. They communicate effectively with residents, families, and the healthcare team, and provide education on care plans, treatments, medications, and disease processes. This role includes hands-on clinical responsibilities such as managing complex wound care, providing mobility assistance, collaborating on medical procedures, and delivering end-of-life care. The Nurse Practitioner also promotes a learning environment by contributing to staff education, training, and support. As part of the care team, the Nurse Practitioner participates in Quality Improvement/QAPI initiatives, supports a resident-centered care environment, and ensures compliance with all regulatory and safety standards, including reporting requirements. Additional responsibilities include completing discharge summaries and prescriptions, managing prior authorizations, renewing medications, and maintaining accurate billing and documentation. Must be able to work hours as scheduled based on the requirements of the position and department.
REQUIRED EDUCATION AND EXPERIENCE

  • Graduate of an accredited School of Nursing (RN); and
  • Graduate of an accredited Nurse Practitioner Program; or
  • An equivalent combination of education, work, and experience will be considered.
  • Current Advance Practice Registered Nurse (APRN) License.
  • National Board Certification.
  • Current licensure and credentials required by the State of NH to practice as Nurse Practitioner.
  • CPR / Basic Life Support (BLS) certified.
