Position Summary
CarePoint is seeking a dedicated and clinically skilled Nurse Practitioner (NP) specializing in wound care to join our team. The NP will provide on-site wound care services in skilled nursing facilities, working collaboratively with interdisciplinary teams to assess, treat, and monitor patients with acute and chronic wounds.

Key Responsibilities
- Perform comprehensive wound assessments and develop individualized care plans
- Provide treatment for acute and chronic wounds (pressure ulcers, diabetic wounds, surgical wounds, etc.)
- Perform bedside procedures including debridement (as appropriate based on training and scope)
- Collaborate with physicians, nursing staff, and facility leadership to ensure continuity of care
- Educate staff, patients, and families on wound care prevention and treatment
- Document all encounters accurately and timely in EMR systems
- Monitor wound healing progress and adjust treatment plans as needed
- Ensure compliance with all regulatory and clinical standards
Qualifications
- Active Nurse Practitioner (NP) license in Ohio
- Board Certified (FNP, AGNP, or related specialty)
- Wound care experience preferred (SNF or long-term care a plus)
- Wound Care Certification (WCC, CWCN, etc.) preferred or willingness to obtain
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
