REQUIRED SKILLS: Under general supervision, performs professional Occupational Therapy activities consisting of planning, evaluating, establishing and implementing individual and group treatment activities under the direction of a physician.
Bachelor’s Degree in Occupational Therapy.
Required Certifications & Licensure:
-Current license from the Oklahoma State Board of Medical Licensure and Supervision as an Occupational Therapist.
-Current Basic Life Support issued by the American Heart Association required upon hire.

Work History: Should not be more than 30 days of gap in entire work history
References: 1 Reference in entire work history
