Pharmacist

Job Summary

Provides pharmaceutical care services in assigned clinic areas. Designs, implements, monitors, evaluates, and modifies patient pharmacotherapy to ensure that medications are indicated, safe, and effective. Completes documentation and billing processes for pharmacist activities. Participates in protocol-driven practice as appropriate. May enter into collaborative practice agreements with physician if meets qualifications. Participates in educational and scholarly activities and educates patients, healthcare professionals, and students regarding medication therapy. Participates in quality improvement and resource management initiatives, including but not limited to HEDIS Measures. Participates in and supports research activities of the department and clinic areas served. Supports the continuous improvement, patient safety, and various accreditation programs and initiatives of the department and institution.

Qualifications

Education

  • Required
    • High School Diploma, GED, or equivalent
    • Bachelor's degree in Pharmacy
  • Preferred
    • Doctorate degree in Pharmacy

Experience:

  • Preferred
    • Two years of experience in a comparable pharmaceutical setting

Certification/Licensure:

  • Required
    • Registered Pharmacist
  • Preferred
    • Board Certified in Ambulatory Care or Pharmacotherapy
      • Required if forming Collaborative Practice Agreement with physician
