Requirements: 
 • Graduation from an accredited College of Pharmacy with a Doctor of Pharmacy (Pharm.D.) degree 
 • Current Oregon Pharmacist license 
 • American Heart Association Basic Life Support (BLS) Certification 
 • American Heart Association Advanced Cardiovascular Life Support (ACLS) Certification (must be obtained within 6 months of hire)
 • Board Certified Pharmacotherapy Specialist (BCPS) certification through the Board of Pharmacy Specialties (BPS), or other relevant BPS certification - preferred
 • Completion of ASHP accredited residency or minimum of 3 years of relevant hospital experience 
 • Oregon license
 • BLS (AHA)
