What you'll do:
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide skilled therapy services in patients' homes
- Re-evaluate progress and adjust care plans as needed
- Collaborate with interdisciplinary care team members
- Educate patients and families on treatment goals and techniques
- Complete clinical documentation in accordance with agency standards
Compensation & support:
- Hourly pay range: $39.25 - $60.84 (based on experience)
- 6-month housing bonus (conditions apply)
- Relocation bonus up to $10,000 (restrictions apply)
