Overview:
Our home health team is seeking a Physical Therapist to provide high-quality rehabilitative care to patients throughout the Palm Springs area. This role offers the opportunity to deliver individualized therapy services that support recovery, mobility, and long-term independence in a home health setting.

Position Offers:

• Full-time Monday–Friday schedule
• Competitive salary of $135,000–$146,000 annually based on experience

Key Responsibilities:

• Conduct patient evaluations and develop individualized treatment plans
• Implement therapy interventions and monitor patient progress
• Educate patients and caregivers on exercises, safety, and recovery strategies
• Maintain accurate, timely, and compliant clinical documentation
• Collaborate with interdisciplinary care teams to support patient outcomes
• Support patients and families throughout the rehabilitation process
• Participate in ongoing training and professional development

Qualifications:

• Doctor of Physical Therapy (DPT) with active California Physical Therapy license
• Minimum of 1 year of experience, preferably in home health
• Strong knowledge of rehabilitation techniques and care planning
• Experience with electronic health record systems
• CPR certification preferred
