Location: Granville Physical Therapy

Position Overview:
The Outpatient Physical Therapist evaluates patients and their environment to include: medical history, strength, motor development, mental status, sensory perception, mobility, functional capacity, respiratory and circulatory efficiency and safety issues. Performs evaluations and treatments in the outpatient setting. Maintains an environment of safety for patients, self and others. Records findings to develop or revise treatment programs. Develops plan of care based on past medical history, evaluation of patient data, patient/caregiver goals and physicians' orders. Provides therapeutic exercises, manual therapy, neuromuscular reeducation, therapeutic activities and administers physical agents. Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit. Documents evaluations, treatments, patient response and progress in patient's chart. Confers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate physical therapy treatment with other aspects of patient's health care. Communicates effectively with patients, families and other staff members. Provides oversight to Physical Therapist Assistants in relation to patient plan of care. Participates in performance improvement activities and committees as assigned.

Key Responsibilities:
• Provide clear and timely documentation of evaluation, treatment planning and treatment activities to ensure quality care.
• Demonstrate competency skills related to outpatient physical therapy.
• Provide patient /caregiver education according to the best learning style of the individual.
• Demonstrate professional oral and written communication skills to other team members and referral sources.
• Be comfortable serving as a resource person in areas of expertise for both Granville Health System and the local community.
• Actively seeks out opportunities for advancing clinical knowledge through professional association memberships, continuing education workshops and/or professional networking.
• Be able to work independently

Qualifications

Licensed to practice in the state of North Carolina. Will accept responsibility for maintaining clinical practice skills. Will adhere to Medical Center policies and procedures. Experience in outpatient orthopedics and occupational health preferred.
