PHYSICAL THERAPIST

Summary:

Adheres to principles consistent with Physical Therapy for the evaluation and treatment of delays and conditions of individuals ages birth-geriatrics.

Treats individuals, ages birth-geriatrics, within a clinic setting, who have been referred for PT

Adheres to the services in accordance with the measurable result/outcome/goal, frequency, duration, beginning and end dates outlined in the Treatment Plan of Care

Adheres to documentation standards established by Personalized Therapy and submits all session notes, outcome summaries and other reports as deemed necessary in a timely manner

Maintains a patient visit schedule consistent with the needs of the families an company guidelines, while eliminating unnecessary travel

Works with the patient's family/caregiver(s) to provide strategies they can work on to address the individuals's delay

Helps patient accomplish treatment plan and accept therapeutic devices by administering manual exercises; instructing,encouraging, and assisting patients in performing physical activities, such as non manual exercises, ambulatory functional activities, and daily-living activities and in using assistive and supportive devices, such as crutches, canes, and prostheses

Administers PT treatments by giving massages, initiating traction, applying physical agents, utilizing hydrotherapy tanks and whirlpool baths, moist packs, ultraviolet and infrared lamps, and ultrasound machines; directing treatments given by aides, technicians, and assistants

Evaluates effects of PT treatments and fit of prosthetic and orthotic devices by observing, noting and evaluating patient's progress; recommending adjustments and modifications

Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferences

Assures continuation of therapeutic plan following discharge by designing home exercises programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs

Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions' calling for repairs

Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions

Treats individuals of all race, creed, gender, and disability without bias or prejudice

Respects and understands the child/family's right to services and right to involvement in service decisions

Practices ethical responsibility based on the cultural, ethnic and religious beliefs of the patients serviced and applicable law

Prepares and submits reports, assuring confidentiality and following all HIPPA guidelines.

Attends meetings as required.

Communicates effectively with other members of the multidisciplinary team.

Keeps abreast of new techniques and trends in the field of PT

Attends conventions, meetings, or special courses whenever possible and to share with the clinic personnel information gained

Prepares and gives talks to community groups and participates in community outreach events in support of Personalized Therapy's mission, as requested

Maintains good public relations with all community sources

Plans for future improvement in procedures and organization of the program

 

 

Holds current license as a Physical Therapist in the state of Maryland

Current (or ability to obtain) First Aid and CPR/BLS Card (infant/child)

Criminal background check - cleared

Valid drivers license and reliable transportation

Excellent customer service, interpersonal and communication skills

Excellent documentation skills

Ability to troubleshoot; strong sense of decision making and judgement; action oriented; approachable

Team player and effective at building and fostering teamwork as well as maintain composure when dealing with conflict; thoughtful of others, courteous

Active listening skills and able to adapt to change

Effectively manages time and priorities
