2 year experience, State license, BLS
Facility Setting? Acute Rehabilitation Unit
Number of beds in your facility? 12
Preferred experience level? Inpatient rehab experience
EMR for your facility: Meditech
Scrub colors / dress code: Black
Parking Location: NE parking lot


Required Qualifications
Physical Therapist Assistant, Rehab
Licenses: PT-KS
Certifications: BLS-AHA(Copy needed)
AND
Physical Therapist Assistant, Acute
SSN Required
DOB Required
Work History: Should not be more than 30 days of gap in last 1 year
References: 1 Reference in entire work history
