Essential Functions:
- Manage medical chronic and acute conditions effectively in collaboration with a physician or specialty provider
- Perform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
- Ensure that all diagnoses are ICD9/ICD10 coded accurately and documented appropriately to support the diagnosis
- Ensure that all quality elements are addressed and documented
- Conduct a post-hospitalization medication reconciliation
- Facilitate agreement and implementation of the member's plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians
- Utilizes practice guidelines and protocols established by the facility
- Care Coordination
- Assess the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members
- Coordinate care as member's transition through different levels of care and care settings
- Continually monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change
- Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the member's needs and wishes
- Evaluate plan of care for cost effectiveness while meeting the needs of members, families and providers to decreases high costs, poor outcomes and unnecessary hospitalizations
Knowledge, Skills and Abilities:
- Ability to formulate a differential diagnosis
- Proficient in assessment and development of treatment plans
- Ability to conceptualize the needs for the team or department and actualize plans for implementation
- Ability to develop plans of action and gain team consensus towards common goals
- Familiarity with ICD-9 coding
- Intermediate level of technical proficiency in the use of computer programs for medical records or internet search for clinical practice
- Ability to complete clinical care lifting in a provider office environment
- Caring and compassionate professional committed to offer exceptional service and quality care to our patients in our Assisted Living Facilities, Group Homes, and Sober living housing facilities. This includes traveling to the facility location, with the opportunity of freedom and flexibility and extra earnings you may not find in a typical 9-5 .
Position Type: Part-Time, Full-Time and Contract
Education and Experience:
- 1+ year of experience as a Nurse Practitioner or Physician Assistant
- National Nurse Practitioner certification in Adult Medicine, Family or Gerontology or ability to obtain, or Physician Assistant certification or ability to obtain
- APRN or PA-C Licensure in Nevada or ability to obtain
- DEA or ability to obtain prior to employment
- Experience working with patients with chronic health conditions, behavioral health issues, and community-based care.
- A background in managed care
Additional Qualifications:
- Must be able to pass a drug and background check.
