Psychiatric Advanced RN Practitioner - Posting #22-183 MAJOR DUTIES: This position is the lead provider responsible for caring for our patients in the secured withdrawal unit located in the Crisis Stabilization Center at Lifeline. The primary duties include conducting psychiatric assessments; assessing medical health needs (with consultation available by our medical team); making appropriate referrals to community physicians and health facilities; and providing management and administration of medication in conjunction with the psychiatrist and medical team. This position also includes providing clinical mental health services to agency patients. In fulfilling these duties, the incumbent performs the following duties: * Conduct complete history and physical exams, including patient-centered comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical medical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships. * Provide service coordination for patients including participation with the individual treatment teams (ITT) * Document patients progress in the electronic health record according to established methods and procedures. * Participate in court proceedings, to include: staffing, testimony, patient advocacy, and writing petitions. * Participate in staff meetings and treatment planning review meetings. * Provide medication management and request consultation when appropriate for medical care * Organize the distribution of medication to patients consistent and congruent with a patients' individual treatment plans. * Assist in ordering pharmaceuticals and medical supplies for the unit. * Provide integrated care services by assisting in coordinating, scheduling, and conducting (when appropriate) medical assessments of patient's physical health, making appropriate referrals to community medical centers/providers as well as the LLC primary care team for further assessment and treatment, and coordinate psychiatric treatment with medical treatment, including medication assisted treatment. * Provide ongoing and updated assessment of patients' mental illness symptoms and patients' response to treatment while in the program. Update treatment plans to ensure standard of care and evidence-based treatment is provided. * Educate patients in self identifying symptoms related to their mental health disorder and the natural course of their illnesses. * In addition to medication management, provide behavioral health to patients on an individual, group, or family basis in the office and in community settings which teaches behavioral symptom-management techniques to alleviate and manage symptoms that promote personal growth and development by assisting patients to adapt to and cope with internal and external stresses. * Provide individual treatment in an integrated care model that is non-confrontational, recognizes co-occurring mental health disorders and substance use disorders, and uses a patient centered approach to establish goals. * Promote holistic integrated care by, encouraging and supporting patients to obtain a primary care physician, dentist, and other medical specialists. * Must be committed to supporting people with severe mental health illnesses in their right to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services. Advocate for building skills and competence to establish supportive trusting relationships with persons with severe and persistent mental health disorders and respect for patient rights and personal preferences in treatment are essential. * Must understand the co-occurrence of mental health disorders and addiction and be willing to assist people with both including providing Medication assisted treatment in the acute and outpatient care setting. * Support and implement agency policies and procedures. * Other duties as assigned.
