Registered Nurse
Citizens Memorial Hospital (CMH) in Bolivar, MO is hiring a Registered Nurse (RN) to join our Care Transitions Team! CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!
About the department: Improving patient outcomes by facilitating care provided within the Patient Centered Medical Home and providing support through transitions in care.
A typical day includes: Receiving and handling patients' medical-related telephone calls, relaying information to patients/ others as directed by the provider or resource, documenting in the medical record information relayed.
- Assists schedulers and patients in determining the appropriate level of care for medical need and assists with facilitating access.
- Enters or responds to EMR task appropriately and timely.
- Appropriately report diagnostic results, medication information or changes and information as instructed ensuring that patient/caregiver questions are answered and instructions understood.
- Assist patients with medication questions and issues including coordination of provider orders to pharmacy as indicated in the medical record.
Job Requirements:
- Education: RN degree from an accredited program of study.
- Experience: Healthcare or Customer Service Experience
- Skills: organization, able to work in a fast-paced environment, multitasking
- Licensure/Certification: Active RN license in the state of Missouri
- Physical: ability to work at a computer 90% of day and stand for the remaining 10%
The position is Full-Time, days 8:00 a.m. - 4:30 p.m.
