Pay Rate: $3131.52 - $3451.06
- Department: Outpatient case management callback program
Responsibilities: This case manager will be calling on patients discharged from the hospital with a specific diagnosis of sepsis. Case management will be done telephonically. Candidate needs to be computer literate, someone who is comfortable navigating electronic health records and to type proficiently as they will be documenting and ordering through the patient’s chart. Inpatient, acute care background, strong clinical skills. Able to communicate clearly. All work will mostly be telephonic and through the computer however will be in the office. Knowledge of home health and ordering equipment. Will be scheduling appts.
