POSITION DUTIES Care Coordination / Continuum of Care / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS RN License / BLS / Acute Care Experience

LENGTH OF ASSIGNMENT 13 weeks

SHIFT / HOURS PER WEEK Days 8-430 / 40

START DATE ASAP

SYSTEMS Interqual / Meditech

Facility Location
